ex. 3 hours for 16 weeks = 48시간 tutorial, seminar 등의 instructor가 함께하는 강의 경우, 총 수업 시수 (lecture hours) 옆에 따로 기입 ex. 48hrs +seminar 20hrs

Consent Form for Collection and Use of Personal Information for Credit Transfer

1. 개인정보의 수집 항목(Personal information to be collected)

- 필수항목: 성명, 학번, 소속 대학, 소속 학과, 휴대폰 번호, 이메일, 파견학기, 파견국가, 파견 대학, 전공여부

Required information: Name, student ID number, college, department, cell phone number, email, exchange period, exchange country, exchange university, Business major type and etc.

2. 개인정보의 수집 목적(Purpose of collecting personal information)

- 교환학생 외국대학 이수학점 검토 및 학점인정 처리

Review and transfer of exchange students’ credits earned from exchange universities

3. 개인정보의 보유기간(Storage and period of use)

- 수집된 개인정보는 준영구로 보유합니다

Collected personal information shall be stored semi-permanently

4. 동의를 거부할 권리 및 동의 거부에 따른 불이익(Right to deny)

- 귀하는 상기 동의를 거부할 수 있습니다. 다만 이에 대한 동의를 하시지 않을 경우에는 교환학생 과목검토시 어려움이 있을 수 있습니다.

You have the right to deny the collection and use of personal information. However, there could be difficulty in transferring course credits from the exchange university.
